Guidelines for Checking Out Material from the Best Practice in Supported Employment Library

  1. All requests for materials must be sent to Randy Phillips in writing by mail, e-mail, or fax:

  2. Please include the following information: Name of person requesting information, name of organization, street address (not PO Box #), telephone and fax number, and email address.

  3. Material(s) will be shipped out as soon as possible. Person requesting the materials will be responsible for shipping the items back to the above address. Items must be returned in original condition and packaged in a reinforced, cushioned envelope.

  4. If materials are returned damaged, recipient could be responsible for replacing the item(s) and barred from using the resource library in the future.

  5. Materials will be loaned out for a two-week period unless a written request is approved in advance. This request should be addressed to Brenda McComb at address listed above. The two-week period will begin two days after shipped from Tuscaloosa.

  6. Only two items can be requested at a time.

  7. Individuals and agencies are asked not to make copies of the materials.
